If setting it up the way you have described below:
User wants to edit/update their selections so the form gets rendered with a
checkbox for all available options and the additional year field.
when a user selects say 5 options and enters in the 5 input fields 5 records
are saved to the ProfileSkill db. User goes back and chooses to not select 1
of the options originally selected, unlike the normal HABTM update where it
deletes all records and then saves them with this set up I have to do the
delete / updating of records myself correct? If so is it bet to do it the
same way HABTM would do it delete all the user records and then save the new
info?

IMO your whole problem stems from trying to refer to a model as a join
table. When a join table has more than 2 fields - make it a model. If you
give profile_skill a primary key, create a model for it, define Profile
hasMany ProfileSkill etc. you've find your difficulties pretty much
dissolve.
