Ah put up the answer a bit too fast.   Here's a better version, where I changed...

Line 275 in /lib/Cake/Routing/Dispatcher.php from... 

$fileFragment = implode(DS, $parts);

to...

 $fileFragment = urldecode(implode(DS, $parts));
