You should NOT need to change anything in the file app/index.php. You have not given enough information about you installation for us to help. For instance, where (in relation to your webserver document root) did you put CakePHP. If you did a standard installation and put it in the document root directly, just go to <domain_name> and you should see the the CakePHP welcome screen. It will tell you from there what needs to be done next (like setting up your database connection).
-- If you don't see the welcome screen, and given that you are on a shared server, you probably don't have mod_rewrite configured on the server correctly. Try looking here: http://book.cakephp.org/2.0/en/installation/url-rewriting.html
The installation instructions at http://book.cakephp.org/2.0/en/getting-started.html are really very good and you should follow them. BTW, they say nothing about modifying app/index.php. Follow the instructions and you will get it done.
