You need the form to be converted as fields and everything else to pdf?
This will be very hard to do, trust me.
I can recommend you that you do this:
After client fill out the form, you post this data to another .php file that will create one html file formatted just with simple html and css,
so the html2pdf will print it very nice with the information provided in the form.
I just know this way, I made tons of stuff like that and always worked

Hi,
I have to convert a very complex form to pdf.
I have tried html2pdf opensource but its not working. Its ofr very simple conversion.
My client is also not accepting coversion from web2pdf.
I have tried to convert using fpdf but form is complex and it seems that it cant be done with it. here is the link of form

Its pdf but i have to fill this form dynamicall so thats why i have to convert this form from html 2 pdf.
Any help guyz. I need this to be done on my server.
