I'm trying to create a multi-page survey, and kind of unsure which
route or direction to go, can someone point me in the right direction?

Here is what I'm looking for:
** 2 types of questions, single choice and multiple choice.
** 1 question, per-page.
** It would be really nice if there was a way to have self-entered
answers for some of the questions.
** Logging -  Log when the question loads for them and when they
answer the question (start and end times).
** Logging - Capture and store their ip address - to help with bots/
abusers. I have an app that I created doing something similar now, but
I use hidden fields and I'm sure there's a better way to do that.

If there is something already built in cakephp that will do this that
I've somehow overlooked, I'd very much appreciate someone pointing me
in that direction.

As always, thank you for any help, it is very much appreciated.
